Change Account Owner Form
Complete this form to transfer all your right, title, and interest in your VT529 account to a different Account Owner. You should consult a qualified financial, legal and/or tax advisor regarding the application of federal, state, and local tax law as a transfer of account ownership may have adverse tax consequences. Custodial Account Owners are subject to special limitations on their ability to transfer ownership of the account.

Fill out this form to make a rollover from another 529 College Savings account, a UTMA/UGMA Account*, Coverdell Education Savings Account or qualified U.S. Savings Bond.

Fill out this form to make a rollover from your 529 educations savings account to a qualified Roth Individual Retirement (IRA) account.
*If you roll funds from your VT529 account into a Roth IRA, you’ll forfeit your Vermont state tax credit proportional to you received the dollar amount of the rollover.
